Yes, I did all that. In fact, I have been submitting a ticket several times already since 4 years ago and there seems to be nobody is looking into it. My tickets were removed without even sending me an email nor a notification that my ticket has been responded to or not.
So, I tried re-submitting a request for review/removal on November 6, 2015 (last year), but my request ended up to nothing again. This was the ticket # being submitted: Ticket ID #1113783. And just recently, I submitted another request in hopes that McAfee will attend to it this time.
Here are my new tickets:

My domain is now clean from any malware issues and malicious records/blacklists.
