McaFee says that powershell is the Threat Source Process

McaFee says that there is a generic trojan threat in my server, the source process is "C:\Windows\System32\WindowsPowerShell\v1.0\powershell.exe",  which try to access a target file xxx.exe in my server. what does this mean? does it mean that powershell.exe is infected?
