I am trying to add company Quickbooks files to Excluded Files under Antivirus/Real-Time Scanning. Some files work, others won't. How do I add the ones that won't work with Add File? And does this even work?

McAfee security software for Windows or macOS, such as LiveSafe or Total Protection, allows you to exclude individual files from being scanned for viruses by the product's Real-Time, Scheduled, On-Demand, or command line scanning features. Kindly refer the below article for details.

As I had already explained in my original explanation, I could not add most of my Quickbooks files to the excluded files list. They just wouldn't load. Quickbooks gave me their solution, which was to hold the Alt key down when trying to open a Quickbooks file. That is supposed to turn off the scan for that file. However, it only works part of the time.

Can you give a specific example?
One possible explanation is that you are trying to exclude a shortcut file (.lnk) and the product is adding an exclusion for the target file rather than the shortcut.
Are you trying to exclude files because they are getting detected? If so, what is the threat name listed under Quarantined Items?
You mentioned holding the Alt key down works part of the time. What does it looks like when it doesn't work vs when it does?
