I don't know whether you ever got this resolved through McAfee Technical Chat, but the comments about their support operation that I've seen so far on this forum make me doubt it...

You need to run the PVReader application from the command line, not by double clicking on it.

Be warned that this program will save your passwords to your disk in plain text (not encrypted).  You should copy the passwords to a safe place and then shred the file (don't just delete it) as soon as possible after running the program.  Unfortunately it looks like McAfee have removed the shredder with this "upgrade" as well as the password vault, presumably just to make absolutely sure they lose our custom.  But you can find free altertanives easily enough - just google "file shredder".
